Have you tried Millers/Forte additive or similar? I've read numerous reports they work but may need repeated use to make sure its cleaned properly. Millers for injectors - Forte for turbo veins/EGR. From that i've read - sticking turbo veins, sticking egr and maf sensor can cause the issue.
